Diabetes Educator

 

Position Type: full-time on-site | Position Status: open until filled | Position Start Date: 10/23/26

Reports To: Executive Director and/or Diabetes Quality Coordinator

MINIMUM QUALIFICATIONS AND EDUCATION REQUIREMENTS:

Bachelor’s degree or higher in Nursing, Dietetics, or Pharmacy.

Must have a active, in good standing, professional license or eligible for licensure.

Must have a valid Drivers License.

Experience: Atleast two years experience in diabetes education is preferred.

Position Summary:

The Diabetes Educator delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes and cardiometabolic care. The DE is an integral part of the interprofessional team and provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan. The DE supports and advocates for people affected by diabetes to optimize quality care. The DE promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.

Job Description

ROLE AND RESPONSIBILITIES:

  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice.

  • Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardiometabolic conditions.

  • Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.

  • Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes and cardiometabolic conditions across the lifespan.

  • Contributes to research and applies current research and evidence-based care to practice.

  • Applies business principles, systems practice, and population health management to support achievement of the Quadruple Aim (reduced costs, better outcomes, improved experience, and improved work life for healthcare providers).

  • Applies the standard seven self-care behaviors to educate on and initiate behavior change.

  • Engages in lifelong learning and serves as a role model of professionalism.

  • Provides quality diabetes self-management education and medical nutrition therapy (if applicable) in individual and group settings based on assessed needs.

  • Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.

  • Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines.

  • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’s assessed needs and goals and promotes successful self-management.

  • Documents all individual contacts/visits in the Electronic Health Record and outcomes database according to the guidelines in a timely manner.

  • Collaborates, advocates, and confers other members of the diabetes care team in developing person-centered diabetes plans.

  • Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.

  • Collaborates with community partners to establish and maintain on-going support options.

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.

  • Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.

Certified Community Health Worker

Position Type: full-time on-site | Position Status: open until filled | Position Start Date: 11/01/26

Reports To: Executive Director, Diabetes Educator and/or Diabetes Quality Coordinator

MINIMUM QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • High school diploma or equivalent.

  • Completion of an approved Community Health Worker training program or willingness to obtain CHW certification within 30 days of employment.

  • Must pass a background check and fingerprinting.

  • Must hold a valid Drivers License.

  • Reliable transportation.

  • Must complete a minimum of 15.0 continuing education credits related to diabetes annually.

Job description

The Certified Community Health Worker (CCHW) serves as a trusted health professional within Southern New Mexico Diabetes Outreach (SNMDO), supporting individuals and families affected by or at risk for diabetes, healthcare providers, and community partners. The CCHW supports SNMDO’s mission by providing culturally responsive health education, outreach, care navigation, referrals, follow-up, and support to individuals living with diabetes, prediabetes, and those at increased risk for chronic disease.

The CCHW will work closely with SNMDO staff and dedicated volunteers to increase access to diabetes prevention and education services, improve participant engagement and retention, address social and community barriers to health, and strengthen connections between community members and appropriate healthcare and social-service resources. The CHW will balance community-facing responsibilities with administrative and program support duties necessary to ensure accurate participant documentation, effective program coordination, data collection, and successful delivery of SNMDO services.

  • Provide administrative support for SNMDO programs and daily operations as assigned

  • Phone and fax operations

  • Assist with participant enrollment, registration, intake forms, consent forms, and program paperwork

  • Maintain organized and accurate electronic and paper records in accordance with SNMDO policies

  • Enter participant, screening, referral, attendance, and program data into designated databases or tracking systems

  • Assist with scheduling classes, appointments, meetings, support groups, and community outreach activities

  • Assist with preparing program materials, educational packets, participant folders, sign-in sheets, certificates, and other program documents

  • Prepare and organize materials and supplies for classes, health screenings, community events, and outreach activities

  • Assist with ordering, organizing, and maintaining program supplies and inventory

  • Respond to routine participant inquiries by phone, email, or in person and direct questions to the appropriate staff member when needed

  • Assist with preparing correspondence, reports, spreadsheets, presentations, and other administrative documents

  • Assist program staff with tracking attendance, participant engagement, referrals, and program outcomes

  • Support grant-related documentation and data collection as assigned

  • Assist with collecting information needed for grant reports, audits, quality improvement activities, and program evaluations

  • Maintain calendars and assist with coordinating program activities and staff schedules as directed

  • Assist with meeting preparation, including agendas, materials, sign-in sheets, and follow-up documentation

  • Perform routine filing, scanning, copying, mailing, and other office support duties

  • Maintain professional communication with participants, community partners, vendors, and SNMDO Board and staff

  • Report to the SNMDO Board of Directors at regular meetings

  • Assist with maintaining a welcoming, organized, and professional office and program environment

  • Perform other administrative and program-related duties as assigned by the Executive Director

    Collaboration

    The CHW will collaborate with:

    ● SNMDO staff including; admin, Diabetes educators, Lifestyle Coaches

    ● Occasional collaboration with healthcare providers and referral partners, community-based organizations, schools and youth-serving organizations
