Diabetes Risk Test

Participant Eligibility:
Must be at least 18 years old or older.
Not currently pregnant.
No diagnosis of type 1 or type 2 diabetes.
Body Mass Index (BMI) of 25 or higher.
Must have at least one of the following:
Have a confirmed diagnosis of prediabetes with a recent hemoglobin A1C test result between 5.7% to 6.4%.
Have a history of gestational diabetes.
Have a high-risk score (typically 5 or above) on the risk test.

If your risk test score was above 5, or if you don’t know your most recent A1c result, schedule your screening appointment with Southern NM Diabetes Outreach as soon as possible. Call 575-522-0289 to schedule.
