National Diabetes Prevention Program (NDPP)

The National Diabetes Prevention Program (NDPP) offered by Southern New Mexico Diabetes Outreach is a 12-month, evidence-based lifestyle program designed to help people reduce their risk of type 2 diabetes through modest, achievable, changes in diet, physical activity, and behavior.

NDPP combines group support with lifestyle coaching, personalized goal-setting, and regular tracking of weight and activity to aim for optimal weight loss to lower diabetes risk.

The program centers culturally relevant education, practical skills, meal planning, stress management, and strategies to overcome barriers to healthy living.

Participant Eligibility:

  • Must be at least 18 years old or older.

  • Not currently pregnant.

  • No diagnosis of type 1 or type 2 diabetes.

  • Body Mass Index (BMI) of 25 or higher.

    Must have at least one of the following:

  • Have a confirmed diagnosis of prediabetes.

  • Recent A1C test result between 5.7% to 6.4%.

  • Have a history of gestational diabetes.

  • Score a 5 or higher on the risk test.
